Grace Therapy provides evaluations and therapy to clients across the state!
Step One
Reach out to us! In order to start the process, complete a contact form so we can learn more about your child.
Step Two
We’ll need a referral from your child’s pediatrician. We’ll also need our intake paperwork to be completed.
Step Three
Let’s get you scheduled! Once we receive a referral and the completed paperwork, we’ll schedule your child’s initial evaluation.
Insurance & Billing
At this time, we only accept private pay.
